VNA Health Care is a patient-centered, not-for-profit healthcare provider, serving nearly 90,000 patients a year. VNA is certified as Primary Care Medical Home (PCMH) by The Joint Commission. The PCMH Certification focuses on care coordination, access to care, and how effectively a primary care clinician and interdisciplinary team work in partnership with the patient (and when applicable, their family).

We offer free pregnancy testing and if you are pregnant, we’ll help you apply for Medicaid and get started with great prenatal care. Babies and children come for vaccinations, sick care and physicals (including for school).

Adults visit for general care and care when they are sick. Patients with diabetes and high blood pressure get the care they need to manage symptoms and stay healthy. All ages have access to primary care including mental health services and low-cost prescription drugs through VNA.

We welcome patients with Medicaid, Medicare, private insurance or no insurance at all. If you don’t have insurance, you may qualify for a discount based on income and family size.
